Medical Release Form: If you are planning on attending any event with CYM, you must fill out a current 2008 Medical Release Form. Take just a moment to fill it out and turn it in at the CYM Booth on Wednesday nights, Saturday nights, or Sunday Mornings.
Adult Staff Application: Are you interested in joining our CYM volunteer adult staff? Take a moment and fill our our Adult Staff Application and turn it in at the Youth Booth after any of our weekly services or to either Pastor Zach or Pastor Mark.
Chaperone Application: If you are a parent or volunteer who is interested in going on a trip with CYM, please fill out our CYM Adult Chaperone Application. This doesn't mean you want to be on the full-time volunteer staff. It simply means you might want to help out with one of our big events like Summer Camp, Mission Trips, or Spring Retreat. You can turn your application in at the Youth Booth after any of our weekly services or directly to Pastor Zach and Pastor Mark.
Scholarship Application: If you need financial assistance with any CYM event, please fill out a Scholarship Application and turn it in at the Youth Booth. One of the Youth pastors will call you and determine the level of assistance that is available.
Update Form: If any of your personal information has changed recently, please take a moment and fill out an update form. We want to know how to get in touch with you and keep you well informed of all that is going on at CYM.
